Volunteer Requirements

These forms are required to be submitted for all volunteer roles. Please print and complete the forms and return to the Parish Office.

Apply for a blue or exemption card

To apply for a blue or exemption card digitally, follow these 3 steps.

1. Have your Government CRN nearby to reference. If you have a drivers license your CRN is the same as your License number and can be found on the top right corner of your drivers license as shown in the picture below

2. Register for an online account. This is required to verify your identity and get the photo for your card to link with your bluecard.

3. Once registered - Apply for your bluecard or exemption card using the online applicant portal

   Alternatively if you wish to apply using a paper form download the form here. Print it, complete it with your details, and then return it to the office.
